1) Patients not actively receiving opioids/opiates or using low dose (less than 30 mg morphine equivalents daily [MMED])
 
OR
 
2) Patients actively on 30 to 80 MMED

 2) recommended 1st and 2nd titration Rx's 
 3) proposed timing for follow-up
